Your Report

Instructions

Once the SCIO hair analysis test is purchased we will email you with a health questionnaire to fill in. You will need to print this questionnaire off, alternatively if you don't have access to a printer you can email back once completed.

You will need a small lock from the head, about 1” in length which will go in its own envelope in order to avoid contamination. You will place the questionnaire in the outer envelope along with the smaller envelope which contains the hair sample and post to the address given in the email. We will email an order number along with the health questionnaire, please quote this on a piece of paper inside the envelope.
